Dr. E. Bill worked as the internal wrangler for the Santa Monica Medical Clinic since 1999, and was involved in the secret activities of the Medical Clinic's Blood Bank, draining captive thin-bloods to sell the vitae to Kindred.

• Dr. Bill normally doesn't appear in the game. However, if suckhead kills Phil Flabottomus before the scene with Lily, then Phil's replaced in the later scene with Dr. Bill. The name is was likely chosen due to its phonetic similarity to "Phil," as Vandal Cleaver's line saying "there's one less Phil in the world" is otherwise identical, except for the changed word "there's one less Bill in the world."
• His title is a reference to the album Internal Wrangler by the band Clinic.
• Assuming he's in his residency at the medical clinic, this might make him "Resident E. Bill," a pun on the Resident Evil franchise.
