- Holotype: MZUSP 29672, adult ♂, skin, skull and postcranial skeleton, collected by Ana Paula Carmignotto on 8 May 1997.
- Type locality: “30 km to the north of the Gaúcha do Norte municipality, between the Coluene and Curisevo river basins, in northeastern Mato Grosso state, Brazil (13°00’53”S; 53°10’39”W [Datum SAD 69], 359 m.a.s.l.)”.
