- Holotype: MUSA 7427, adult ♂, skin, skull, mandibles, tissues, and partial skeleton, collected by K. Pino on 20 October 2009.
- Type locality: “Miraflores, San Ignacio, Cajamarca, Peru (5°12’ S and 79°12’ W) at 2,800 m above sea level”.
