- Holotype: ANSP 12732, ♀, skin and skull, collected by Samuel Nicholson Rhoads on 12 May 1911.
- Type locality: “Hacienda Garzon, Mt. Pichincha, 10,500 ft. [= 3200 m] altitude”, Pichincha, Ecuador.
