- Holotype: QCAZ 52006, adult ♂, collected by S.R. Ron, F. Ayala, T. Camacho, M. Yánez, D. Rivadeneira, S. Aldás and D. Pareja on 19 September 2011.
- Type locality: “Ecuador, Provincia Tungurahua, Caserío Machay, 3 km E of Río Verde on the road to Puyo, (1.3923°S, 78.2801°W), 1349 m above sea level”.
