- Holotype: FMNH 123917, adult ♀, body in alcohol with skull removed, collected by David E. Willard on 18 October 1981.
- Type locality: “Consuelo, km 165, 17 km by road west of Pilcopata, Province of Paucartambo, Department of Cuzco, Peru, approximately 13°07'59' [sic] S, 71°15'W”.
