- Holotype: MSB 70717, adult ♀, skin, skull, and postcranial skeleton, collected by Jorge Salazar-Bravo on 15 July 1994.
- Type locality: “Ecuador, Provincia de Bolívar, Río Tatahuazo, 2.5 km NE de Cruz de Lizo (–1.716667, –78.98333, WGS84 coordinates taken by GPS at the collection site, elevation 2,875 m)”.
